3. Select the "Click to Save" button on the PDF form. Or select File -> Save As PDF from Adobe Reader.
4. Save the completed .PDF file to your desktop, or other memorable location.
5. Enter your company name in the field below.
6. Below, choose your form type from the drop down menu.
7. Select "Choose File" and select the .PDF file from your desktop, or other location.
8. Click "Submit" and your PDF form will be sent.
